Your Role

 

In this diverse and rewarding role you as part of the Rendu House Complex Support Needs Program, you will support indviduals with co-occurring mental health concerns, long histories with the criminal justice system, complex trauma and AOD use. Through specialist support coordination, therapeutic intervention and case management, you will help clients stabilise, engage in treatment and achieve positive change. You will work within a skilled, collaborative team and contribute to the ongoing development of the service. 

Your Responsibility

 

  • Deliver specialist support coordination and case management via face-to-face, telehealth or community outreach

  • Deliver the Finding a Balance (FAB) program, a psychoeducation program delivered to clients who may be unable to complete a structured Day Program

  • Provide evidence based therapeutic interventions aligned to the AOD Model of Care

  • Complete intake, assessments and risk screening

  • Develop and review client centred treatment plans

  • Support clients to achieve their goals

  • Liaise with external services to ensure coordinated care

  • Engage families and significant others where appropriate

  • Maintain accurate case notes and records

  • Participate in supervision, case reviews and team meetings

  • Contribute to service quality, planning and continuous improvement

You Will Need

 

  • Tertiary qualification in Social Work or Psychology

  • Experience supporting people with complex needs including AOD, mental health, homelessness or justice involvement

  • Strong communication and relationship building skills

  • Commitment to ethical and non judgemental practice

  • Knowledge of community services and referral pathways

  • Able to work independently and within a multidisciplinary team

  • Well organised with basic computer and client system skills

  • Unencumbered NSW driver’s licence
