Your Role
Make a meaningful impact by supporting individuals on their recovery journey at Adele House. In this role, you’ll provide high-quality case management and therapeutic interventions in a positive and therapeutic environment, improving health, wellbeing, and social outcomes for clients. As an AOD Case Worker, you’ll deliver tailored support and interventions in line with the Adele House Model of Care. Your focus will be on fostering recovery and empowering individuals to build brighter futures.
Your Responsibilities
- Provide holistic case management from intake to discharge, including assessments, care planning, and risk management.
- Facilitate group programs, provide psycho-social support, and assist clients in resolving conflicts.
- Support clients transitioning into and out of residential services, ensuring continuity of care.
- Engage with families and significant others to strengthen client support networks.
- Collaborate with multidisciplinary teams, including nurses, counsellors, and support staff, to achieve positive outcomes for clients.
- Conduct drug and alcohol screenings, assist with medication self-administration, and maintain accurate records.
- Contribute to on-call rosters, quality improvement initiatives, and accreditation activities
You Will Need
- Relevant qualifications in Alcohol and Other Drugs (Certificate IV or Diploma), Community Services, or a related field.
- Experience managing caseloads for clients with complex needs, including AOD issues, mental health concerns, homelessness, or criminal justice involvement.
- Skills in delivering strengths-based, recovery-oriented approaches.
- Strong teamwork, communication, and organizational skills.
- Proficiency in data collection and reporting systems.
- A valid and unencumbered NSW driver’s licence.
