- Office Based in Newcastle + WFH Flexbility
- SCHCADS Award LV5 hourly rates range from $51- $53.31 (dependent on experience)
- Maximum-term full-time contact until June 2026, in line with current funding.
- Generous Not-for-Profit Benefits increasing total pay package up to 16k.
Your Role
In this rewarding role you will be providing care coordination, intensive case management and specialist support for clients who are seeking assistance with alcohol and other drug (AOD) use and other complex support needs. You will undertake person-centered comprehensive assessments and care planning in conjunction with the client and use your knowledge and skills of the AOD and other sectors to effectively provide tailored care, that assists individuals achieve their care plan goals.
In this diverse role you will be delivering services on an outreach basis, meeting with clients in various locations, onsite and virtually.You will be joining a supportive and inclusive team, where you can contribute meaningfully to the community and make a real difference to people's lives.
Your Responsibility
- Undertake person centred comprehensive assessment & care planning in conjunction with persons seeking support.
- Work in partnerships with referring staff and agencies.
- Develop and implement referral pathways & facilitate access to range of community-based health and non-health services.
- Ensure treatments are targeted & culturally appropriate for Indigenous Australians including linkages to appropriate services.
- Maintain accurate client records, case notes and other data as directed and required.
- Provide direct support and referral in the particular focus areas of employment, training/education, daily living skills, homelessness/housing, social engagement and participation and family and community reconnection.
You Will Need

Relevant tertiary or VET sector qualification with substantial experience

Proven experience coordinating care for individuals with complex needs, including AOD issues, mental illness, homelessness, trauma, justice involvement and family or cultural separation

Skilled in assessment using standard tools

Ability to provide therapeutic interventions

Professional, ethical and non-judgemental conduct
- Confident working independently and able to take initiative

Knowledge of and ability to access relevant community resources

Capable of working both independently and as part of a team

Proficient in microsoft office and comfortable with data entry

NSW driver licence
