SCHADS Award Level 5 hourly rates from $51 to $53.31

Full-time, maximum-term contract until June 2026 (aligned with current funding contract)

Your Role
In this diverse and rewarding role, you will coordinate care, provide intensive case management and deliver specialist support to clients with alcohol and other drug (AOD) challenges and other complex needs. You’ll complete comprehensive, person-centred assessments and develop tailored care plans in partnership with each individual. Your work will be delivered through a mix of onsite, outreach and virtual support, allowing flexibility to meet the clients where they are.Based in Wagga, you will play a key role in creating meaningful change across the Riverina region.
Your Responsibilities
- Conduct comprehensive, person-centred assessments and develop practical, holistic care plans, that are targeted to individual needs and are culturally appropriate.
- Collaborate with referral partners and other service providers
- Facilitate connections to relevant alcohol and other drug, health and community services
- Support clients to engage and maintain contact with services
- Support clients with implementing their goals regarding drug and alcohol, housing, social participation, family or community and employment.
- Maintain accurate records and documentation
You Will Need
-
Relevant tertiary qualification or VET sector qualification with substantial experience

Experience working with individuals with complex needs, including AOD use, mental health, homelessness and trauma
- Experience in delivering outreach support in a community services/health setting

Skilled in assessment and therapeutic interventions

Respectful, professional and non-judgemental approach

Able to work independently and as part of a team

Strong knowledge of local community resources

Competent in Microsoft Office and confident with data entry

Current NSW driver licence
